Phase 5 Re-opening

SPECIAL COVID-19 ANNOUNCEMENTS

Starting May 24, 2021 we will be in Phase 5 of our re-opening plan.

​

PHASE 5 HIGHLIGHTS:

​

• Patients and clients can choose to no longer wear masks 2 weeks after being fully vaccinated (2 doses of Moderna or Pfizer and 1 dose of Johnson & Johnson vaccines). 

​​

• Proof of vaccination is NOT required.  Instead we'll rely on the honor system. 

​​

• Please speak with Lisa, Practice Administrator, or Dr. Corn if you are uncomfortable being around individuals not wearing masks.

​​

• We reserve the right to ask any patient or client to wear a mask for medical reasons, including if he/she presents with any cold or flu-like symptoms. 

​​

• For your protection, all employees will continue to wear  masks when patients and clients are present.
